We consider referrals from other healthcare providers to be the highest form of compliment a practice can receive. We truly appreciate the confidence placed in our team each time another healthcare provider refers one of their patients to our office.
Please click on the appropriate link below for the type of referral you are making to our office, and fax the completed form to our office at 417.883.5577. Upon receipt, we will contact the patient to schedule an appointment, and we will notify your office of the date/time. If you do not hear from us within one business day, please call our office to verify we received your fax.
Please note that we will also require a copy of the patient’s last office note, lab results, and any cardiac testing results he/she may have had performed.
